Fall Sports Registration

Our Fall Athletic Program is open to all 7th and 8th Grade students attending Sells Middle School. The season begins at the beginning of August and runs through the beginning of October. The following sports are offered: Football, Volleyball, Girls and Boys Golf, Coed Cross Country and Girls Tennis.

Please see the steps below on how to sign up your student-athletes for the Fall sports season. Physicals and Final Forms account creation must be completed by August 1st. 

REGISTRATION PROCESS

#1- Final Forms. Create a Final Forms account at https://dublin-oh.finalforms.com. Both the parent AND student-athlete must create a Final Forms account AND sign all necessary forms for the Fall. The account needs to be created and all forms need to be signed by 8/1/2022. Make sure you specify the sports your child plans on trying out for/playing on Final Forms. If your child plans on trying out for a cut sport, you can sign your child up for that sport AND a non-cut sport (as a back-up plan). This will ensure you are on the coaches' e-mail group. Coaches will contact all those who sign up by mid-to-late October to inform you of any details regarding practice/try-outs. #2- OHSAA Physical. Schedule a Physical Exam with your family physician. Have him/her complete the required OHSAA Physical Form. These are valid for (1) calendar year. A valid form needs to be submitted to Athletic Director Trey Jones or Head Trainer Pam Stotts by 8/1/2022. For an OHSAA Physical Form click here. For an OHSAA Physical Form en espaniol, click here.

#3- COVID Waiver. Please make sure you complete and sign the Dublin City Schools COVID Waiver Form for athletics. The hard copy needs to be printed, completed, signed, and given to the Head Coach on the first day of open gym or open mats.

#4- Wait. After you complete all of the information above, the coaches will send an informational e-mail in mid-to-late October complete with the practice/try-out schedule and reminders.

SPORT SPECIFIC INFORMATION

For sport specific information, such as try-out practice/game schedules and coach contact information, click on "Spring Sports" in the left navigation menu. This information will be uploaded to the web-site no later than mid-February.

COVID GUIDELINES

  • The Dublin City Schools COVID Waiver is a requirement for your child to participate.

  • If your child is experiencing COVID symptoms (fever, cough, runny nose, shortness of breath, etc), please KEEP YOUR CHILD HOME and contact the coach.

  • No water will be provided. Your child must bring his or her OWN water (at least 32 ounces). No exceptions.

  • Please have your child bring his or her own hand sanitizer for during practice.
