Transition Services

Planning a child's transition from adolescence to adulthood is one of the key steps in preparing them for a successful future. In Ohio, transition planning for students with disabilities begins at the age of 14, or earlier if necessary.

The mission of Dublin Schools' Transition Services is to promote the integration of students with disabilities into both their school and community by offering a range of opportunities tailored to support each student's unique needs, interests, and strengths. Through the transition planning process, families and students are introduced to valuable resources that will assist them as they move from school to post-school activities. The transition services are coordinated by IEP team members, who work with families and students to develop a personalized transition plan that aligns with the student’s goals for postsecondary education, employment, and independent living/community integration.

  • Assistance with agency referrals
