Health and Clinic Information
The Dublin City School District employs eight school nurses who rotate between their assigned schools each week. There is also a full-time clinic aide in each building who, together with the nurse, monitors the health care of students in the district.
Immunizations
Immunization records must be provided to the school. After records are reviewed, you will be
contacted if additional vaccinations are required. Students without documentation of all required immunizations by the 15th school day will be excluded from school until the appropriate vaccinations are obtained and documentation submitted to the school health clinic.

Minimum Immunization Requirements for Kindergarten Students in Ohio
- DTaP (diphtheria, tetanus, pertussis): Four (4) doses if 4th dose was given on or after the 4th birthday and at least 6 months after the 3rd dose. If all 4 doses were given before 4th birthday, a 5th dose is required.
- Polio: Three (3) or more doses of IPV. The FINAL dose must be administered on or after the 4th birthday with at least six months between the final and previous dose, regardless of the number of previous doses. If a combination of OPV and IPV was received, four (4) doses of either vaccine are required.
- MMR (measles, mumps, rubella /German measles): Two (2) doses of MMR. Dose 1 must be administered on or after the 1st birthday. The second dose must be administered at least 28 days after dose 1.
- Hepatitis B: Three (3) doses. The second dose must be administered at least 28 days after the first dose. The third dose must be given at least 16 weeks after the first dose and at least 8 weeks after the second dose. The last dose in the series (third or fourth dose) must not be administered before age 24 weeks.
- Varicella (Chicken pox): Two (2) doses of varicella vaccine must be administered prior to entry. Dose 1 must be administered on or after the first birthday. The second dose should be administered at least three (3) months after dose one (1); however, if the second dose is administered at least 28 days after first dose, it is considered valid.
Emergency Medical Authorization
Please provide us with all available contact numbers and include individuals authorized to pick up your child if you are unavailable. Please update contact information throughout the school year if changes occur. In the event of an illness or emergency, we will release your child ONLY to those listed as an emergency contact. This MUST be updated each year. Medical information provided will be shared with appropriate staff in case of an emergency. Please contact your school nurse prior to or at the beginning of the school year if your child has a medical condition such as a life-threatening allergy, diabetes, seizures, or other serious concerns, to ensure your child’s medical needs are met at school.

- ALL medication to be administered by school personnel, including nonprescription/over the counter medications, requires a parent/guardian authorization AND an Ohio licensed prescriber’s signature. New paperwork must be completed and on file each school year for each medication. Cough drops and sunscreen are exceptions to this requirement. Elementary level students will need written permission from parents for cough drops, and they must be stored in the clinic.
- Parent/guardian must provide medication to the clinic and it must be transported to and from the clinic by an adult. The only exception is for high school students with private transportation.
- All medication to be administered by school personnel must be in the original container as dispensed by a physician, pharmacist, or manufacturer and will be stored in a locked compartment in the school clinic.
- All medications must be picked up on or before the last day of school. Any medication left in the clinic will be discarded.
- Students may self-carry select emergency medications according to their medical needs:
- Students with Asthma may self-carry a prescribed inhaler with “Asthma Action Plan and Orders” (Form 5330A A F1) completed and on file in the school clinic.
- Students with life threatening allergies may self-carry a prescribed Epinephrine auto-injector with “Allergy and Anaphylaxis Emergency Orders and Action Plan” (Form 5330A E F1) completed and on file in the school clinic. Please note, a second epinephrine auto-injector must be supplied to the school clinic.
- Students with Diabetes may self-carry prescribed medications with “Consent for Diabetic Management” (Form 5330 F2) completed and on file in the school clinic. Please note it is highly recommended that a second glucagon kit be supplied to the school clinic.
Exclusions
Students experiencing any symptoms associated with a communicable illness will be excluded from school in accordance with the recommendations set forth by the Ohio Department of Health (ODH) and/or Franklin County Public Health (FCPH). State and local health guidelines may be subject to frequent changes as directed by health officials. Please note, regardless, students must be symptom free without the assistance of medication for 24 hours before returning to school. If a student is prescribed antibiotics (including eye drops) for a contagious illness, he/she must be on the medication for at least 24 hours before returning to school even if they are without fever or other symptoms. Please report any contagious illness to the clinic or office.
Vision and Hearing Screening
All Kindergarteners will have vision and hearing screenings completed before November 1st. You will be notified if your child does not pass these screenings, and a recommendation will be made for further evaluation by a specialist.
