In order to register an athlete for any sport, please complete the following steps and make sure you visit the individual sports page for their specific information:
1 - Complete Final Forms (see information below) (please see individual sports for info also)
2 - Complete a physical and submit (see information below)
3 - Pay the District Pay to Play fee (see information below). This is generally not active until the sport season starts. If you are trying out for a "cut" sport, do not pay the fee until after try outs and you have made the team (Girls volleyball, girls tennis, boys golf, girls/boys basketball, softball, cheerleading, and baseball)
4 - Please be patient. After you have completed all of the above, the coaches will send out an informational email to the group with any and all relevant information for their sport.
5 - Visit the KMS athletic website frequently to see if any updates have been made.

Final Forms
Create a Final Forms account at https://dublin-oh.finalforms.com. Both the parent AND student-athlete must create a Final Forms account AND sign all necessary forms for the Fall. The account needs to be created and all forms need to be signed. Make sure you specify the sports your child plans on trying out for/playing on Final Forms. (This may or may not be up and running - please see individual sports for info also). If your child plans on trying out for a cut sport, you can sign your child up for that sport AND a non-cut sport (as a back-up plan). This will ensure you are on the coaches' e-mail group. Coaches will contact all those who sign up to inform you of any details regarding practice/try-outs.

OHSAA Physical Forms
It is required for each athlete to have an OHSAA Physical Form on file BEFORE they can participate. Turn in to Athletic Director Christian Miller or Head Trainer Paul Dawson. Physicals are valid for (1) calendar year.

Pay to Participate Fee
Dublin City Schools Middle School Athletics bear a $30 per season, per sport Pay to Participate fee. This fee is paid electronically. Click here to pay your fee:
